These plans are developed and overseen by a consultant in pain medicine and aim to simplify medication regimens, reduce unwanted side effects, and improve overall function and quality of life.

For patients using long-term pain medicines — particularly opioids — a tailored review can identify what is helping, what may be less effective, and how best to make adjustments in a safe, supported way.

How the Procedure Works

Chronic pain is complex, and over time many people accumulate multiple medications in an attempt to control symptoms. However, higher doses or multiple drug combinations do not always provide better relief and can lead to fatigue, low mood, weight gain or reduced clarity, amongst others.

Analgesic rationalisation involves carefully reviewing your current pain medications, their benefits, side effects, and interactions. Your consultant will then work with you to devise an optimised plan focused on what truly helps your pain and daily function.

For patients taking opioids (such as morphine, oxycodone, fentanyl, or tramadol), opioid de-escalation may be appropriate. This process involves gradually and safely reducing opioid use under medical supervision. The aim is to minimise withdrawal symptoms and help your body readjust while introducing alternative treatments to support pain management.

Throughout this process, your consultant works closely with you — and, if appropriate, your GP or pharmacist — to ensure changes are made at a pace that feels manageable.
