Understanding Brachialgia

Brachialgia refers to pain that radiates from the neck into the shoulder, arm, or hand — typically caused by irritation or compression of one of the nerve roots in the cervical spine (neck). It is the upper limb equivalent of sciatica, and is often described as a sharp, burning, or electric pain following the path of a nerve down the arm.

Typical symptoms include:

  • Shooting, burning, or aching pain from the neck into the arm or hand
  • Tingling, pins and needles, or numbness in the fingers
  • Muscle weakness or loss of grip strength
  • Neck stiffness or pain aggravated by head movement

The most common causes include cervical disc herniation (slipped disc), age-related spinal wear (spondylosis), or facet joint changes that narrow the nerve exit channels (foramina). In some cases, muscle tension or postural strain may worsen symptoms. Because brachialgia can resemble shoulder or tendon problems, expert diagnosis is vital to determine whether the pain originates from the neck or elsewhere.

Wessex Pain Clinic

How We Diagnose

At Wessex Pain Clinic, our consultants carry out a thorough history and examination, assessing posture, spinal movement, and neurological function (strength, sensation, and reflexes).

To identify the cause precisely, we often use MRI scanning to visualise the cervical discs, nerves, and surrounding structures.

Wessex Pain Clinic
Lifestyle and Complementary Therapies

Successful recovery depends on restoring normal neck function and movement. Our physiotherapists focus on posture correction, cervical mobility, and strengthening exercises to support long-term stability. Hydrotherapy can provide gentle, low-impact rehabilitation for patients with significant stiffness or pain.

Our pain psychology team helps patients manage chronic symptoms, stress, and sleep disturbance, using techniques such as pacing and mindfulness. When required, we coordinate care with neurosurgical and rehabilitation specialists to ensure comprehensive treatment.

At Wessex Pain Clinic, we work as a team and meet regularly to discuss complex, treatment resistant or unusual cases. As such you will often receive the expertise of several pain consultants. We believe that we are one of very few pain clinics to offer this.

Medications

Depending on the nature and severity of pain, we may prescribe:

  • Anti-inflammatory medicines (NSAIDs) to reduce swelling and inflammation
  • Neuropathic pain medications such as gabapentin, pregabalin, or duloxetine for nerve irritation
  • Short courses of muscle relaxants or stronger analgesics under consultant supervision

Medication plans are reviewed regularly to optimise effectiveness and minimise side effects.

Interventional Procedures

For persistent or severe pain, our consultants perform image-guided cervical nerve root blocks or epidural steroid injections to reduce inflammation and calm the irritated nerve. Sometimes we will refer you to one of our partner interventional radiologists to have this done.

In complex cases, we collaborate closely with spinal surgeons to assess whether decompression or disc replacement surgery is appropriate.

Our treatment for brachialgia is multidisciplinary and patient-centred, combining medical therapy, interventional pain techniques, and physical rehabilitation to relieve symptoms and prevent recurrence.
