Understanding Endometriosis

Endometriosis is a chronic condition in which tissue similar to the lining of the womb (endometrium) grows outside the uterus — most often on the ovaries, fallopian tubes, bladder, bowel, or pelvic walls. These patches of tissue respond to hormonal cycles just like the womb lining, thickening and bleeding each month. Because this blood has nowhere to go, it causes inflammation, scarring, and adhesions, leading to persistent pelvic pain and other symptoms.

Endometriosis can significantly affect quality of life, mood, relationships, and work. While it cannot always be cured, specialist treatment can dramatically reduce pain, improve fertility outcomes, and restore day-to-day function. We work alongside the regions best gynaecologists to provide coordinated care for endometriosis.

Wessex Pain Clinic

How We Diagnose

At Wessex Pain Clinic, our consultants take a comprehensive history focusing on pain patterns, menstrual history, bowel and bladder symptoms, and prior surgeries. If indicated and appropriate, we may recommend an examination

Diagnosis often involves collaboration with gynaecologists and pelvic health specialists, as symptoms may overlap with other causes of pelvic pain such as bladder pain syndrome, irritable bowel syndrome, or musculoskeletal dysfunction.

Investigations may include:

  • Pelvic ultrasound or MRI scans to identify cysts (endometriomas) or adhesions
  • Diagnostic laparoscopy (keyhole surgery, performed by gynaecologists)
  • Pelvic floor assessment if muscular tension or nerve pain is suspected
  • Diagnostic nerve or joint blocks (e.g. pudendal) to identify overlapping pain mechanisms

This comprehensive assessment helps build a clear picture of all contributing factors — allowing for a truly personalised treatment plan.

Wessex Pain Clinic
Lifestyle and Complementary Therapies

Our specialist physiotherapists partners provide targeted pelvic floor rehabilitation, posture training, and manual therapy to reduce muscle tension and improve movement.
Hydrotherapy supports gentle, low-impact exercise and relaxation.

Our pain psychology partners help patients manage the emotional and cognitive impact of chronic pelvic pain — addressing stress, anxiety, fatigue, and the effect of persistent pain on relationships and self-confidence. Approaches such as CBT, ACT, and mindfulness-based pain management are used to promote resilience and self-efficacy.

At Wessex Pain Clinic, we work as a team and meet regularly to discuss complex, treatment resistant or unusual cases. As such you will often receive the expertise of several pain consultants. We believe that we are one of very few pain clinics to offer this.

Medications

We may recommend:

  • Hormonal therapies (in collaboration with gynaecologists) to suppress or regulate menstrual cycles
  • Anti-inflammatory medicines (NSAIDs) for pain relief
  • Neuropathic pain medications (e.g. amitriptyline, duloxetine, pregabalin) for nerve-related pelvic pain
  • Topical treatments such as lidocaine or topical gabapentin for local nerve hypersensitivity

All medication plans are monitored carefully for balance of effectiveness and tolerability.

Interventional Procedures

Where pain persists, our consultants offer precise, image-guided interventions to target the pain at its source, including:

  • Pudendal nerve blocks to relieve pelvic floor or perineal pain
  • Sacroiliac joint injections where pelvic joint inflammation contributes
  • Pelvic floor trigger point injections to release muscle spasm
  • Lidocaine infusions to desensitise overactive pain pathways
  • Pulsed radiofrequency (PRF) of key pelvic nerves for longer-term relief
Our care for endometriosis is holistic, multidisciplinary, and patient-focused, addressing hormonal, inflammatory, nerve-related, and musculoskeletal aspects of pelvic pain.
