Understanding Genitofemoral Neuralgia

Genitofemoral neuralgia is a chronic pain condition caused by irritation or injury to the genitofemoral nerve, which arises from the upper lumbar spine and travels through the abdomen to supply sensation to the groin, upper thigh, and genital region.

When this nerve becomes compressed, stretched, or damaged, it can lead to persistent pain or altered sensation in these areas.

Typical symptoms include:

  • Burning, stabbing, or aching pain in the groin, lower abdomen, or upper thigh
  • Pain radiating into the genital area (scrotum or labia)
  • Heightened sensitivity, tingling, or numbness in the affected region
  • Discomfort following abdominal, pelvic, or hernia surgery

Common causes include post-surgical nerve trauma, scar tissue formation, entrapment of the nerve in abdominal muscles, or occasionally lumbar spine nerve root irritation. Because the pain can mimic ilioinguinal or femoral nerve problems, hernia, or hip pathology, specialist assessment is vital for accurate diagnosis.

Wessex Pain Clinic

How We Diagnose

At Wessex Pain Clinic, diagnosis begins with a detailed medical history and examination, including mapping of the pain distribution and identifying surgical or traumatic triggers.

Our consultants perform a focused neurological examination, assessing sensation and tenderness along the genitofemoral nerve pathway.
We may use ultrasound or MRI imaging to exclude structural or surgical causes.

Wessex Pain Clinic
Lifestyle and Complementary Therapies

Physiotherapy goals include restoring gentle mobility and strength in the lower abdomen, pelvis, and hip, while avoiding strain on the affected nerve. Hydrotherapy offers a supportive environment to aid recovery.

We also partner with pain psychology specialists to help patients manage chronic pain, reduce anxiety, and build resilience through pacing and mindfulness strategies.

Our integrated, consultant-led approach ensures that both physical and psychological aspects of pain are addressed for long-term improvement.

At Wessex Pain Clinic, we work as a team and meet regularly to discuss complex, treatment resistant or unusual cases. As such you will often receive the expertise of several pain consultants. We believe that we are one of very few pain clinics to offer this.

Medications

We may prescribe:

  • Anti-inflammatory medicines (NSAIDs) for acute inflammation
  • Neuropathic pain agents such as gabapentin, pregabalin, or duloxetine to calm nerve sensitivity
  • Topical treatments such as lidocaine or capsaicin patches for localised pain
  • Short courses of stronger pain relief.
Medication plans are regularly reviewed to ensure effectiveness and safety.
Interventional Procedures

Our consultants perform ultrasound-guided genitofemoral nerve blocks, using local anaesthetic and corticosteroid to reduce inflammation and pain.

For chronic or recurrent pain, radiofrequency neuromodulation can provide longer-lasting relief by gently interrupting the overactive nerve signals.

In complex cases involving overlapping pelvic or groin nerves, ilioinguinal or iliohypogastric nerve blocks may also be used.

Whilst usually not required, we work alongside surgeons if it is felt that a surgical opinion is needed.

Our care for genitofemoral neuralgia is multidisciplinary and individualised, focusing on relieving nerve irritation, reducing pain, and restoring function.
