Understanding Migraines

Migraines are a complex neurological condition that can significantly affect work, family life and overall wellbeing. At Wessex Pain Clinic, our consultant pain specialists provide comprehensive migraine diagnosis and treatment for patients across Hampshire, Sussex, Surrey, Dorset, Berkshire, Wiltshire, & the South of England.

Typical symptoms include:

  • Throbbing or pulsating pain, usually on one side of the head
  • Nausea, vomiting, or dizziness
  • Sensitivity to light, sound, or smell
  • Visual disturbances (known as aura) — flashing lights, zigzag patterns, or blind spots
  • Fatigue and cognitive “fog” before or after an attack

There are several types of migraine, including migraine with aura, migraine without aura, and chronic migraine. Triggers can include hormonal changes, stress, certain foods, sleep disruption, or environmental stimuli. Because migraines can have a major impact on work, relationships, and wellbeing, specialist management is often needed to reduce frequency and severity.

How We Diagnose

At Wessex Pain Clinic, diagnosis begins with a detailed medical history and symptom review, including attack pattern, duration, and associated features. Our consultants identify potential triggers and differentiate migraine from other headache types such as cluster headache, tension-type headache, or occipital neuralgia.

If required we will coordinate imaging to rule out other causes of headache. Keeping a headache diary can help track frequency and triggers, guiding a tailored treatment plan.

Lifestyle and Complementary Therapies

Our physiotherapy and rehabilitation specialists help address posture, muscle tension, and movement patterns that may contribute to migraine triggers. Hydrotherapy can support relaxation and recovery.

Our pain psychology and lifestyle coaching partners focus on stress management, sleep hygiene, pacing, and coping techniques. We help patients identify and manage personal triggers — whether hormonal, dietary, or environmental — as part of a holistic, long-term plan.

At Wessex Pain Clinic, we work as a team and meet regularly to discuss complex, treatment resistant or unusual cases. As such you will often receive the expertise of several pain consultants. We believe that we are one of very few pain clinics to offer this.

Medications

Treatment typically includes both acute and preventive strategies:

  • Acute treatments: Triptans (e.g. sumatriptan), anti-inflammatory medicines (NSAIDs), and anti-sickness drugs to relieve symptoms during an attack.
  • Preventive medications: to reduce attack frequency.

Medication plans are closely supervised to ensure effectiveness and minimise side effects. We often see patients who have tried preventative medications, that have resulted in side effects precluding treatments. We offer several alternatives (below) in these situations.

Interventional Procedures

Interventional Procedures

For patients with frequent or treatment-resistant migraines, we offer several targeted procedures:

  • Occipital nerve blocks to reduce nerve irritation and dampen migraine pathways
  • MCNBs (multiple cranial nerve blocks) to tackle more persistent symptoms
  • Botulinum toxin (Botox®) injections for chronic migraine, administered at precise points around the head and neck to prevent attacks (sometimes called PRE-EMPT or PREEMPT)
  • Nerve stimulation or neuromodulation (in selected cases) to control severe, persistent headaches
Our approach to migraine management is comprehensive, personalised, and multidisciplinary, combining medical therapy, interventional procedures, and lifestyle support to reduce attacks and improve quality of life.
