Understanding Pudendal Neuralgia

Pudendal neuralgia is a chronic pain condition caused by irritation or entrapment of the pudendal nerve, which carries sensation from the pelvic floor, perineum, genitals, and anus. When the nerve becomes compressed or inflamed, it can lead to persistent pelvic or perineal pain that is often difficult to localise.

Typical symptoms include:

  • Burning, stabbing, or aching pain in the pelvis, genitals, perineum, rectum, or lower pelvis
  • Pain that worsens with sitting and eases when standing or lying down
  • Tingling, numbness, or hypersensitivity in the pelvic area
  • A feeling of “something being present” in the rectum or vagina
  • Discomfort during or after sexual activity
  • Pain that can disturb sleep or daily activity

Pudendal neuralgia can result from nerve entrapment between pelvic ligaments, childbirth trauma, cycling, pelvic surgery, or chronic pelvic muscle spasm. Because the symptoms can overlap with gynaecological, urological, or colorectal conditions, expert multidisciplinary assessment is essential for accurate diagnosis and tailored treatment.

Wessex Pain Clinic

How We Diagnose

At Wessex Pain Clinic, we take a comprehensive, sensitive, and expert approach to diagnosis.

Our consultants begin with a detailed history, exploring symptom patterns, triggers, and any relevant surgical or obstetric history.

A focused examination, if appropriate, assesses areas of tenderness and nerve distribution.

Where necessary, we use MRI or ultrasound imaging to rule out structural issues and to guide further treatment.

Lifestyle and Complementary Therapies

Because pudendal neuralgia often involves both nerve irritation and muscle dysfunction, we work with specialist pelvic physiotherapists.

Our pain psychology and rehabilitation specialists support patients in managing the emotional and practical challenges of chronic pelvic pain, using pacing, mindfulness, and cognitive behavioural techniques.

This integrated, consultant-led model ensures physical, psychological, and functional recovery are addressed together.

At Wessex Pain Clinic, we work as a team and meet regularly to discuss complex, treatment resistant or unusual cases. As such you will often receive the expertise of several pain consultants. We believe that we are one of very few pain clinics to offer this.

Medications

We may prescribe:

  • Neuropathic pain agents (e.g. pregabalin, gabapentin, or duloxetine) to calm nerve sensitivity
  • Anti-inflammatory medicines (NSAIDs) for short-term relief of inflammation
  • Topical treatments such as lidocaine or compounded creams for local nerve pain

All medication is reviewed regularly to ensure optimal relief and tolerability.

Interventional Procedures
Our consultants perform image-guided pudendal nerve blocks, combining local anaesthetic and corticosteroid to reduce inflammation and interrupt pain signals. In persistent or complex cases, we work closely with specialist pelvic surgeons if we feel surgical opinions are needed.
Management of pudendal neuralgia at Wessex Pain Clinic is multidisciplinary, compassionate, and tailored to each individual. We focus on reducing nerve irritation, restoring pelvic function, and improving quality of life.
