Hip Injection / PENG Block

Hip injections and PENG (Pericapsular Nerve Group) blocks are targeted treatments used to help manage pain arising from the hip joint and surrounding structures. They are performed under ultrasound or X-ray guidance by a consultant in pain medicine to ensure accuracy and comfort. These procedures can be helpful for both diagnostic and therapeutic purposes, reducing pain and improving mobility.

How the Procedure Works

Hip pain often comes from inflammation, arthritis, or irritation of the joint capsule and nearby nerves. A hip joint injection involves the placement of local anaesthetic and a small dose of anti-inflammatory steroid directly into the joint space. This can help to calm inflammation, relieve pain, and confirm whether the joint itself is the main source of symptoms.

A PENG block (Pericapsular Nerve Group block) works slightly differently. Instead of being placed inside the joint, it targets the sensory nerves supplying the front of the hip capsule. This technique can reduce pain without affecting muscle strength, enabling physiotherapy, rehabilitation, or other interventions.

Both techniques are performed with image guidance to ensure precision and minimise discomfort.

What to Expect

The procedure is usually carried out as an outpatient treatment. Your consultant will use ultrasound or X-ray guidance to ensure precise placement of the injection. A local anaesthetic numbs the area to make the procedure comfortable.

If you prefer or require sedation, this can be arranged when the procedure is performed in theatre, under the care of a consultant anaesthetist. Sedation is not available for procedures done in clinic, but we can plan for treatment in theatre if this would make you more comfortable.

Afterwards, you will rest briefly before going home the same day. Some temporary soreness or a heavy sensation in the hip or leg may occur before improvement begins.

Driving advice: You should not drive that day if receiving any sedation. Our procedures do vary on whether it is possible to drive home yourself. Please seek advice from your consultant about driving advice. If you are seeing us for an injection and planning to drive, please check with us beforehand about whether it will be possible to drive yourself home afterwards.

Risks and Considerations

All procedures carry some risks, which will be discussed in detail with you during your consultation. You will have the opportunity to ask questions, and formal consent will always be obtained before any procedure is carried out.
