The pudendal nerve runs from the lower spine through the pelvis and supplies sensation to the perineum, anus, genitalia, and parts of the pelvic floor. If irritated, compressed, or sensitised, it can lead to pudendal neuralgia â a complex and often distressing pain syndrome.
Under X-ray or ultrasound guidance, your consultant places a fine needle near the pudendal nerve as it passes through the pelvis â usually near the ischial spine or Alcockâs canal. A small volume of local anaesthetic, sometimes with corticosteroid, is then injected to reduce inflammation and block pain signals.
The procedure may also be used to confirm that the pudendal nerve is the source of your symptoms.
The procedure is carried out as an outpatient treatment, usually taking 20â30 minutes. Youâll lie on your front, and the area will be cleaned and numbed with local anaesthetic. Your consultant will use live ultrasound imaging to accurately guide the needle to the pudendal nerve.
You may experience a warm or tingling sensation in the pelvic area during the injection. Some numbness in the perineal region is expected and usually temporary. Youâll be monitored briefly afterwards and can usually go home the same day.
All procedures carry some risks, which will be discussed in detail with you during your consultation. You will have the opportunity to ask questions, and formal consent will always be obtained before any procedure is carried out.

Procedures are often most effective when paired with physiotherapy, movement-based exercise, and â where appropriate â psychological support. Driving advice: You should not drive that day if receiving any sedation. Our procedures do vary on whether they are possible to drive home from yourself, please seek advice from your consultant about driving advice. If you are seeing us for an injection and planning on driving, please check with us beforehand about whether it will be possible to drive yourself home afterwards.
