Sacroiliac joint radiofrequency denervation (RFD) is a minimally invasive procedure used to relieve chronic pain arising from the sacroiliac (SI) joints â the joints linking the lower spine to the pelvis. When these joints become inflamed or arthritic, they can cause significant lower back, buttock, or leg pain.
This treatment is carried out by a consultant in pain medicine under image guidance and is designed to provide longer-lasting pain relief than diagnostic or steroid injections alone.
Pain from the sacroiliac joint is transmitted to the brain through small sensory nerves called the lateral branches of the sacral nerves. In radiofrequency denervation, your consultant targets these tiny nerves with precise, controlled radiofrequency energy. The application of heat to these nerves destroys them, and their ability to transmit pain signals. As they only have a sensory function, this treatment does not impact muscle strength or movement.
Before considering radiofrequency treatment, you will typically have undergone diagnostic SI joint blocks using local anaesthetic to confirm that the joint is the main source of pain. If these blocks provide temporary but clear relief, radiofrequency denervation may then be offered as a longer-term option.
The aim is to achieve sustained reduction in pain by targeting the nerves that carry signals from the affected joint. The nerves can slowly regenerate over time, and the procedure may be repeated if necessary.
The goal of radiofrequency denervation is to reduce persistent SI joint pain, improve mobility, and allow greater participation in physiotherapy and daily activities.
While the treatment does not cure the underlying joint changes, it can provide valuable symptom control and improve quality of life.
The procedure is performed as a day-case under sterile conditions using X-ray guidance for accuracy. Sedation is usually offered.
Once the position is confirmed, controlled radiofrequency energy is applied for several seconds at each site. The procedure usually takes around 30â45 minutes.
After the procedure, you will rest before going home the same day. Mild soreness or bruising in the treated area is common and usually resolves within a few days.
Driving advice: You should not drive that day if receiving any sedation. Our procedures do vary on whether it is possible to drive home yourself. Please seek advice from your consultant about driving advice. If you are seeing us for an injection and planning to drive, please check with us beforehand about whether it will be possible to drive yourself home afterwards.
