Temporomandibular Joint Injections

Temporomandibular joint (TMJ) injections are used to manage pain and stiffness in the jaw joint — the small but complex joint connecting your jaw to your skull. These injections are typically offered when symptoms have not responded to self-care, physiotherapy, or dental splints.

How the Procedure Works

The TMJ allows you to open and close your mouth, chew, and speak. Dysfunction of this joint can lead to pain, clicking, locking, or restricted movement — often described as temporomandibular joint disorder (TMD).

In this procedure, your consultant injects a small volume of local anaesthetic, and in many cases, a low-dose corticosteroid, directly into the joint space. This is done using ultrasound or anatomical guidance to ensure accurate placement.

The steroid helps reduce inflammation in the joint, while the local anaesthetic provides immediate (but temporary) pain relief. In some cases, this also helps confirm whether the joint is the primary source of symptoms.

What Conditions It Can Help With

TMJ injections may be helpful for:

Temporomandibular joint disorder (TMD)
Painful or stiff jaw joints
TMJ inflammation due to arthritis or overuse
Locking or clicking of the jaw
Pain exacerbated by chewing, talking, or yawning
TMJ pain unresponsive to splints or physiotherapy
They may also be used diagnostically in complex cases where it’s unclear whether the joint or surrounding muscles are driving symptoms.

Aimed Benefits

The injection aims to:
Reduce inflammation and pain within the jaw joint
Improve jaw mobility and ease of movement
Clarify whether the joint is the cause of your pain
Support participation in physiotherapy or dental management
Reduce reliance on pain medication
Some patients may require repeat injections or follow-up treatments depending on the underlying condition.

What to Expect

This is a short outpatient procedure, usually taking 15–20 minutes. You’ll lie in a comfortable position while the area is cleaned. Your consultant will identify the joint using landmarks or ultrasound and gently insert a fine needle into the TMJ.

You may feel a brief pressure or pop, followed by numbness in the area. Most people can go home the same day and resume light activity shortly after. Chewing soft food and avoiding jaw strain is often advised for the first 24–48 hours.

Risks and Considerations

All procedures carry some risks, which will be discussed in detail with you during your consultation. You will have the opportunity to ask questions, and formal consent will always be obtained before any procedure is carried out.

Combined Approach / Multidisciplinary Care

At Wessex Pain Clinic we find that the best results come from combining medical treatments with active rehabilitation. TMJ injections are often most effective when paired with jaw physiotherapy, dental input (such as bite splints), and — where appropriate — psychological support for managing stress or bruxism. This comprehensive approach supports long-term improvement.

  • Consultant-performed injection into the jaw joint
  • Typically uses local anaesthetic and corticosteroid
